Resources for the Office of Disability Services and the Academic Access Program are listed below. Please note that you must submit documentation in order to receive accommodations.
To make appointments, request a Note Taker, or to request Testing Accommodations, please log in to Accommodate .
- To register for accommodations please submit the Accommodation Request . At the end of this form, you will have the opportunity to upload documentation supporting these requests. You can also submit this documentation to The Office of Disability Services in Carson Hall 500. After successfully completing the registration process, you will be informed of the accommodations that will be provided to you.
- The Office of Disability Services uses a program called Accommodate by Symplicity. This program allows students to requests and manage accommodations, make appointments with ODS staff, as well as request Note Takers and Testing Accommodations.

A Semester Request must be submitted for each College Session (Fall, January, Spring, Summer 1 and Summer 2)
